GMC English Requirements for International Doctors

The GMC accepts more than one way to prove your English
The best-known routes are IELTS Academic and OET Medicine. The GMC may also consider a qualifying primary medical qualification, employer references, a UK job offer or other supporting evidence when its detailed criteria are met.
For many international medical graduates preparing for PLAB or registration, a valid IELTS or OET result remains the clearest route. Evidence accepted for PLAB may differ from evidence assessed for registration, so check the guidance for your exact stage.
Know which GMC English evidence route applies to you
IELTS and OET are not the only possibilities, but alternative evidence is conditional. Do not assume that a degree taught in English, an employer letter or a UK job offer will automatically be accepted.

IELTS vs OET for GMC registration and PLAB
Both tests assess Listening, Reading, Writing and Speaking. The task context, score scale, certificate rules and available preparation support can affect which route is more suitable.
GMC minimum
At least 7.0 in every testing area: Listening, Reading, Writing and Speaking.
GMC minimum
At least Grade B in Speaking, Listening, Reading and Writing in the Medicine version.
Certificate validity
IELTS and OET certificates are normally valid for two years under GMC guidance.
PLAB timing
The certificate must be valid on the day you sit PLAB 1.
Registration timing
It must normally be valid when the GMC approves an application for registration with a licence.
Older evidence
Limited additional-evidence routes may apply, but they are detailed and should not be assumed.
The GMC describes limited routes involving recent PLAB 2 success or evidence of maintained English. Sponsorship applications can have different limits, so confirm the position directly before relying on an expired result.
Which is better for a doctor: IELTS or OET?
Neither test is universally easier. Choose according to the tasks you can perform consistently, every pathway that must accept your result and the preparation support available to you.
IELTS Academic may suit you when…
Reading and writing can cover society, education, science, culture or public issues.
Previous preparation can reduce the time needed to learn the format and task types.
IELTS Academic may also support university or other applications where accepted.
OET Medicine may suit you when…
OET tasks are situated in professional healthcare scenarios.
Speaking and writing preparation can support patient-centred language and correspondence.
Strong feedback addresses purpose, audience, clinical communication and language accuracy.

GMC registration and UKFP English requirements are not the same
The UK Foundation Programme Office is separate from the GMC and states that its IELTS and OET thresholds are higher. A result that satisfies the GMC may therefore be insufficient for a UKFP eligibility application.
Published UKFP 2027 criteria
The UKFP 2027 eligibility application window ran from 1 to 22 July 2026. Applicants preparing for a later entry year must use the guidance for their own application cycle.
UKFP requirements include cycle-specific test dates, result verification and evidence rules. Plan from the highest requirement in your intended pathway.

Overall band scores did not compensate for a lower component. Academic and UKVI Academic versions were accepted under the published rules.
OET Medicine for UKFP 2027
UKFPO used numerical scores, required one test sitting and did not require the qualifying test to be the applicant's most recent sitting.
A doctor applying for UKFP may need a higher result than a doctor using the same test only for GMC registration. Confirm the programme year, test-date window and verification process before booking or retaking.
Passing the test is one milestone, not the whole communication journey
IELTS or OET can demonstrate the required English level. Working safely and confidently in an English-speaking healthcare environment also demands communication under real clinical pressure.

GMC English evidence, IELTS, OET and UKFP
What IELTS score does the GMC require for doctors?
The GMC accepts IELTS Academic, including UKVI Academic, when you achieve at least 7.0 in each testing area and at least 7.5 overall. The scores must be from the same test, subject to the GMC's eligible One Skill Retake route.
What OET score does the GMC require for doctors?
You must take OET Medicine and achieve at least Grade B in Speaking, Listening, Reading and Writing. The grades must be from the same test and the GMC states that they must be from your most recent sitting.
How long are IELTS and OET certificates valid for the GMC?
They are normally valid for two years. For PLAB, the certificate must be valid on the day you sit PLAB 1. For registration with a licence to practise, it must normally be valid when the GMC approves the application.
Can I combine scores from different IELTS or OET sittings?
The GMC requires the minimum scores in the same test. For IELTS, one eligible One Skill Retake may be accepted when one area of a full computer-based test missed the minimum and the retake meets the GMC's rules. OET Grade B results must be in the same sitting.
Are GMC and UK Foundation Programme English requirements the same?
No. UKFPO sets its own criteria and states that its IELTS and OET thresholds are higher. For the published UKFP 2027 cycle, IELTS required 7.5 in every skill and OET Medicine required 400 in every skill.
Is IELTS or OET better for an international doctor?
Neither is automatically easier. OET uses healthcare scenarios and profession-specific tasks, while IELTS Academic uses broader academic topics. Compare your baseline performance, deadlines, accepted routes and available feedback before choosing.
Does passing IELTS or OET fully prepare me to work in the NHS?
A test result demonstrates an important English level, but clinical practice also requires handover, escalation, patient-friendly explanations, checking understanding, teamwork, interview communication and listening across different accents.
